This form is to gather information to provide a *brief* summary about mentors that potential protegés will be able to view, and that the program coordinators will use to match mentors with students. We have a sample profile you can read before you fill this out.

When you are matched with a youth, this profile will serve as a good starting point for conversation.

Some of these questions are the same as the ones you filled out on your application. However, students do not have access to that information. Also, we hope you will keep the students in mind as you write your answers here.
